Preventive Care

One of the best things you can do to stay healthy as an adult is to see a primary care provider regularly. Annual checkups give you a chance to get to know your provider, ask important health questions, give your provider a chance to get to know you and your health history, and to identify minor medical concerns before they become larger problems. Preventive care may include wellness exams, screening and immunization recommendations, cancer, diabetes, high blood pressure, cholesterol, and other screenings.

Sick Visits

Our goal is to provide a same-day sick visit when you have an acute problem such as a fever, common cold, flu, strep throat, cough, etc. If a same-day appointment is not possible, you will be offered our first available time. We offer lab tests on-site which makes it possible to quickly diagnose your illness and start treatment if necessary. Acute & Chronic Medical Care

If you have a chronic medical condition, you need a primary care provider who can monitor your condition and work with you to improve your health. At STAT Primary Healthcare, we do more than offer primary care to our patients. We strive to work with every patient to help them achieve a healthy, sustainable lifestyle. One of the most important, and often neglected, aspects of this is diet. Your diet impacts virtually every piece of your overall health, from physical attributes like weight and body mass index to long-term health outcomes like heart disease or cancer. Our nutrition counseling is tailored to your specific health needs and meant to assist with weight management, diabetes/prediabetes, heart health, gastrointestinal concerns, and emotion and disordered eating.

Office Hours & Appointments

Our office is available from 8:30am - 4:30pm Monday, Tuesday, Thursday, and 8:30am - 12:00 noon Friday. We are closed on Wednesdays. We may be reached to schedule an appointment at (931) 982-6333. You may also request an appointment online through our online request form. One of our staff members will return your call during regular operating hours. If you need an appointment, prescription refill, or test results, please call during regular business hours.

You will receive a reminder of your appointment four days prior and on the day of your appointment by phone, text, or email, depending on your preference. We ask that you cancel or reschedule appointments at least 24 hours prior to you scheduled appointment time. In most cases, same-day appointments are available for urgent medical needs..

Patient Forms & Downloads

For your convenience, you may complete new patient forms in advance. Please select and print from the options below, fill them out completely and bring them with you to your appointment. If you are not able to complete the forms before your visit, please allow at least 30 minutes for completion in the office before you see the provider. We recommend that you:

• Bring your insurance card.
• Bring all current medications in original bottles.
• Please arrive a few minutes early to fill out paperwork. At your request, we can also send you our New Patient Information Packet in advance to fill out and bring to your first appointment.
• Bring a copy of any important prior records if you are transferring your care from another doctor.
• Note that if a co-pay is required by your insurance company, it is due at the time of your visit and may be paid in cash, by check or with a credit card.

Insurance Providers

We participate in most insurance plans, including Medicare. If you are not insured by a plan we do business with, payment in full is expected at each visit. If you are insured by a plan we do business with, present your current card at your office visit so that we may verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage.

Amy Harris Family Nurse Practitioner Amy is a 2017 graduate of the Family Nurse Practitioner program at Tennessee State University in Nashville, Tennessee. She is board certified through both American Nurses Credentialing Center and American Association of Nurse Practitioners. As the owner of STAT Primary Healthcare, Amy specializes in family practice treating patients aged 2 years old through adults of all ages. Amy started as a LPN, where she was Valedictorian of her 2011 class, and has since progressed through the ranks, graduating with her Master's in Nursing, to where she is today.
